We are seeking a motivated Outpatient Physical Therapist to join our team of passionate healthcare professionals dedicated to rehabilitative excellence. As a vital member of our outpatient services team, you'll be responsible for implementing physical therapy plans that meet the individual needs of our patients, helping them achieve their maximum potential for mobility and recovery. Perform initial patient evaluations to assess physical condition and establish personalized care plans Implement treatment plans using techniques such as exercise, manual therapy, equipment, and education Regularly assess and document patient progress, adjusting treatments as necessary Educate patients and their families about their conditions and the steps they can take to augment their rehabilitation Collaborate with other healthcare professionals to create a cohesive and comprehensive care plan Maintain patient confidentiality and handle patient records with discretion Proficiency in modern physical therapy techniques and equipment Excellent interpersonal and communication skills Ability to handle sensitive information confidentially Strong organizational and time management skills Medical Insurance Dental Insurance Vision Insurance Valid Physical Therapist license in New Jersey CPR/BLS Certification
