of resources. Provides service delivery and compliance with
external review agencies. Offers ongoing support and expertise
through comprehensive assessment and care planning. Implements care
plans and evaluates individual patient needs. Enhances the quality
of patient management and satisfaction. Promotes continuity of care
and cost-effectiveness through integrated functions. Acts as a
patient advocate through resource utilization and discharge
planning. Addresses the holistic needs of the patient. Ensures
