Job Description:
Primary Function: Jefferson s Structural heart program works to
provide excellent care to patients affected by significant heart
valve disease (aortic stenosis, mitral stenosis and/or mitral
regurgitation) and patients with structural defects of the heart
such as atrial septal defect and/or patent foreman ovale. Our
interventional cardiology team offers procedures such as
transcatheter aortic valve replacement (TAVR), transcatheter edge
to to edge mitral valve repair (Mitraclip device), and trans
catheter closure of ASD and PFOs. A team based model of decision
making and delivery of care is used to determine eligibility for
these novel procedures. The Structural Heart Program APP will work
interdependently with other health care professionals to provide
quality health care to this patient population. This position will
be exclusively to assist with Jefferson Structural Heart Program s
multidisciplinary team. The APP will work closely with the Lead
Structural Heart Nurse Practitioner and clinical administrative
assistant as part of a team based model and delivery of care.
Essential Functions
- Independently evaluate and treat structural heart patients in
follow up
- Assist with needs of JUP cardiology practice as directed by
Lead NP and/or Cardiology physician partners
- Assist in all combined multidisciplinary team clinics for all
structural heart patients
- Assist in seeing inpatient consults
- Provide independent follow up with structural heart patients
via telehealth delivery model of care as deemed appropriate.
- Receive and direct phone calls with appropriate intervention
and/or delegation to team members based on context/ acuity of
call
- Answer patient questions regarding coordination of care.
- Complete frailty examination upon new patients visits specific
to structural heart program
- Establish and maintain longitudinal relationships with patients
in structural heart program.
- Provide patient instructions for upcoming procedures and/or
tests.
- Ensure patient satisfaction with follow up quality of care
discussions with patients
- Assist in making hospital discharge follow up calls with
appropriate intervention and/or delegation of concerns to
appropriate team member.
- Participate in multidisciplinary structural heart team meetings
for patient care planning purposes
- Knowledge of policies/procedures applicable to assigned
duties
- Interacts with co-workers, visitors, and other staff consistent
with the iSCORE values of Jefferson. Qualifications
- CRNP, ACNP or ACGNP certified
- Prior cardiology experience preferred
- Enthusiastic individual with team based mindset
- Excellent interpersonal skills with focus on customer service
and closed loop communication
- Must be detail oriented and highly organized
- Knowledge of patient care and examination procedures
- Ability to maintain confidentiality at all times Jefferson
